provides one-on-one assistance to individuals moving from a nursing home, or
other residential institution, back into the community.

- Coordinate services and supports.
- Train for personal assistance management.
- Provide information on rights and responsibilities.
- Offer access to independent living skills training.
- Act as a long-term resource.
